For the 2025-2026 school year, please review the following information regarding the Home School Program, including the new online registration process:
- Completed, signed and notarized AFFIDAVIT OF SUPERVISOR OF HOME EDUCATION PROGRAM

- Education objectives
- Evidence of Immunizations or religious exemption
- Evidence of health and medical services
- Special Education notification (if applicable)

IMPORTANT DEADLINES

JUNE 30th - By June 30 of each school year, the supervisor of the home education program must submit the qualified home education evaluator’s certification stating that an appropriate education is occurring for the school year under review. The certification must be submitted by June 30th of each year.
- AUGUST 1st – If you intend to continue in the Home Education Program, or if your student will be new to Home Education for the coming school year, please enroll online using the link above and submit all required documents within the portal prior to August 1st.
